Aims
Improve clinical outcomes for General Medicine (GM) patients in the Emergency Department (ED) by offering early review and intervention.
Background
Many GM patients were waiting for prolonged periods in the ED, delaying care and increasing the risk of complications.
Methods
Implemented an early review process with GM consultants in ED, where patients were seen early to address potential issues and avoid unnecessary admissions.
Results
Reduced average length of stay (ALOS) by one day and expedited management of patients with early interventions.
Conclusion
The early intervention model showed significant improvement in clinical outcomes and reduced ED wait times, making it a sustainable practice.
Lessons Learnt
Early intervention and coordination with other departments were essential for optimizing care and reducing ED congestion.
Additional Information
Since 2022, all the specialties within the umbrella of General Medicine and the Stroke unit have started reviewing their patients in A&E. Geriatric Medicine will soon do the same too. By 2023, we will start a Medical Unit in A&E, where it will be staffed by inpatient team of doctors, nurses, and allied health and providing 24/7 services.
