Aims
Improve CRP uptake, reduce unplanned readmissions, and design an early intervention post-discharge service for AMI patients.
Background
Timeliness of follow-up care critical to AMI outcomes, but existing gaps in CRP uptake and medication optimization were identified.
Methods
Introduced a Post-AMI (PAMI) Clinic offering comprehensive care within 2 weeks post-discharge.
Results
Achieved 12.6% reduction in 30-day A&E returns and 10.8% reduction in all-cause readmissions; improved CRP uptake to 38.1%.
Conclusion
Sustained improvements highlight the importance of timely, integrated post-discharge care.
Lessons Learnt
Early intervention reduces readmissions and enhances patient satisfaction in care continuity.
