Aims
Evaluate the impact of the GeriCare Palliative Care Program in reducing ED transfers for nursing home palliative residents.
Background
Nursing home residents were frequently transferred to the ED due to inadequate end-of-life care, contributing to iatrogenic complications.
Methods
Conducted telemedicine consultations for residents planned for ED transfers, integrating Advance Care Planning (ACP), symptom management, and palliative care.
Results
82% of ED transfers were averted after teleconsultation, and a higher proportion of non-ED transfers had documented ACP and comfort care.
Conclusion
The integrated palliative care program demonstrated that proactive management and advanced planning can significantly improve patient care and reduce hospital transfers.
Lessons Learnt
The program’s success highlighted the importance of ACP discussions and early interventions in reducing unnecessary ED transfers.
