Aims
To reduce the rate of Ward 62 patients planned for Weekday AM discharges vacating the bed after 1pm by 25% within 4 months.
Background
A high percentage of patients for discharge were vacating the beds only after 1pm, with data revealing that 43.5% of patients planned for Weekday AM discharges at Ward 62 leave their beds after 1 pm.
Methods
The project involved mapping the current discharge workflow, identifying key issues, and implementing solutions such as providing digital discharge summaries and ordering discharge medications earlier during doctors' reverse rounds.
Results
% of patient vacating beds after 1 pm for weekday discharge at Ward 62 reduced by 33% from 43.5% to 29% within 4 months. Approximately 155 reams of A4 paper saved per annum.
Conclusion
The digital discharge documents have been rolled out to all wards, and the new workflow will be documented in policies and procedures.
Lessons Learnt
The implementation of digital discharge documents allowed the clinical team to focus on patient care instead of administrative tasks.
