Aims
To empower patients to safely perform cardiac rehabilitation (CR) at home with a digital solution that improves access, compliance, and outcomes.
Background
Low CR enrolment rates and logistical challenges (travel, work commitments) increase cardiac event risks, straining healthcare systems. Digitalization during COVID-19 amplified the need for remote solutions.
Methods
Developed Heart-Track™, a mobile app with heart rate sensors, automated exercise prescriptions, remote monitoring, and gamification. Conducted a randomized controlled trial comparing digital CR to center-based CR.
Results
Digital CR was non-inferior to center-based CR for safety and clinical effectiveness. 83% of patients rated Heart-Track™ as satisfactory, reporting improved motivation and convenience. Annual savings: $82,320 in manpower costs.
Conclusion
Heart-Track™ successfully transformed CR delivery, improving access, outcomes, and sustainability. Plans include expanding to manage other chronic diseases and scaling to community networks.
Lessons Learnt
Early patient engagement and tailored guidance were critical. Collaboration among clinicians, IT developers, and patients ensured user adoption.
