Aims
To create a digital version of the discharge huddle board in the electronic medical record system to save time and effort in manually documenting on a whiteboard, and to improve team collaboration and discussions.
Background
The WH healthcare family was using physical magnetic whiteboards to facilitate inter-disciplinary discharge huddles, which required time and effort to maintain and update.
Methods
The digital discharge huddle board was developed in the electronic medical record system with inputs from the ground, and it updates automatically with real-time data pulled from team documentations.
Results
The digital board updates automatically when patients are admitted, transferred, or discharged, reducing duplicate efforts. It provides visual overviews using icons or color codes and improves workflow by minimizing the need to navigate multiple screens.
Conclusion
The digital discharge huddle board has exceeded expectations, becoming a unique feature in WH's informatics journey, and it is a one-time investment for long-term practice.
Lessons Learnt
IT innovation is only useful if ground users appreciate it. More work is needed to improve usability and sustainability. The user committee played a crucial role in maintaining tool usage.
