Aims
Design and implement an algorithm to discontinue Intake/Output (I/O) charting in YCH inpatient care.
Background
Only 13% of I/O charts were discontinued before discharge; significant time spent on unnecessary monitoring.
Methods
Conducted root cause analysis, developed PDSA-based algorithm, piloted, and monitored staff adherence.
Results
I/O monitoring discontinuation within 8 days increased from 13% to 43%; saved 5.6 hours/day in charting.
Conclusion
Algorithm reduced charting time, increased efficiency, and empowered nurses.
Lessons Learnt
Importance of multidisciplinary teamwork and training for successful implementation.
