Aims
1) Increase number of elective procedures done in NHG Health
2) Reduce waiting time for elective procedures
Background
1) The elective workload for NHG is projected to grow by FY27 which would mean that additional Operation Theatres and Endoscopy suites would have to be built / opened to meet the demand.
2) NHG Healths capacity and capability needs to be geared up to serve the unmet need of residents.
3) Urology, Orthopaedics, General Surgery and ENT have long waiting time for Elective Surgery (Source: MOH Bulletin Q12025).
4) To circumvent the above and improve elective care access for our residents, the project brings the subject matter experts together to map out the future state of cross-institutional listing of cases across the NHG Health hospitals to better optimise resources.
Methods
Process Observations and Staff Interviews to establish current state.
Opportunity Statements, Ideation and Swim Lane Diagram to co-create future state.
Results
1) Criteria for cross-listing procedures was established
2) Suitable conditions for cross-listing from Urology, General Surgery, Orthopaedic Surgery and ENT were identified
3) Care model for NHG Health cross-listing was established
4) Enhancements to NGEMR (Epic EMR system) were identified to support new care model
Lessons Learnt
The team gleaned the following insights from analysing the current state:
1) Given the multiple roles surgeons play, manpower and sufficient coverage is a concern if they are away performing surgery at another institution
2) NHG Health hospitals have limited visibility on others charges and costs resulting in difficulty answering queries on Financial Counselling (FC) and cost of surgery
3) Cross-listing is hampered by cumbersome processes
4) Institutions were concerned about equitable access to OTs
5) Lack of familiarity with host institution practices, equipment may hamper surgeons performance
6) Both patients and surgeons did not find cross-listing appealing
