Aims
Create accessibility to preventive health through community nursing and collaboration.
Background
Seniors faced barriers accessing healthcare during the pandemic due to mobility issues and infection risks.
Methods
Introduced nurse posts in Active Ageing Centres (AACs), vaccination via Mobile Vaccination Teams (MVT), and staff upskilling.
Results
Improved seniors' access to healthcare; 414 seniors vaccinated against COVID-19; increased participation rates at THK centres.
Conclusion
Enhanced access to preventive care by transitioning THK centres into socio-health hubs.
Lessons Learnt
Cross-training and job redesign improved service delivery and staff adaptability during crises.
