Aims
To explore contextual factors influencing the planning and implementation of Geriatric Services Hub (GSH) programs.
Background
The GSH aimed to manage frailty in community-dwelling elderly using a multidisciplinary, community-based approach.
Methods
Conducted interviews with 14 program leaders across five sites; used Framework Analysis to identify influencing factors.
Results
Identified critical factors for success: harmonized screening tools, public awareness, collaborative partnerships, and digital data-sharing platforms.
Conclusion
The GSH model effectively addressed frailty management by leveraging local strengths and partnerships; ongoing evolution required.
Lessons Learnt
Sustainable care models depend on clear communication, capacity building, and collaboration with community and healthcare partners.
