Aims
To build a ground-up collective impact program to mobilize community advocates and peer networks to support older persons at risk
Background
Community engagement to support isolated older persons, leveraging peer networks developed from the EMPOWER program
Methods
Developed capabilities to do case finding, risk stratification, and create public awareness through community art programs
Results
429 members of ComSA Club, mobilized community advocates to conduct outreach and form support groups at the HDB block level for seniors at risk
Conclusion
Significant community activation and increasing number of support groups for seniors formed as a result of the program
Lessons Learnt
Empowered community advocates in supporting older persons, helping create a robust outreach program for seniors at risk
