Aims
Evaluate the effectiveness of the Advance Care Planning (ACP) program in improving training, practice, and implementation between 2012-2017.
Background
Nursing home residents increasingly require palliative care, and there was a need to improve ACP processes for NH staff.
Methods
Retrospective study comparing ACP training and implementation between two time periods (2012-2015 and 2016-2017). Key interventions included ACP advocacy, implementation of ACP e-learning, and mentoring of nurses.
Results
Increased ACP completion by 60%, with improved training, and a 10-fold increase in staff participation in training.
Conclusion
The program significantly improved ACP completion rates and staff knowledge, contributing to better end-of-life care for residents.
Lessons Learnt
The integration of ACP discussions into routine care and continued engagement with healthcare professionals are vital to success.
