Conducted door-to-door engagement; profiled residents with AIC Community Screener Tool; held monthly virtual meetings for case referrals.
Results
Flagged 16 high-risk cases proactively; facilitated discussions on 75 complex cases; established One Care Plan across multiple agencies.
