Knee OA is a leading cause of disability globally; the traditional care model lacked coordination and optimisation.
Methods
Multidisciplinary team collaboration, RCT studies, and stakeholder engagement.
Results
Improved clinical outcomes for OA patients with 92% reporting symptom improvement. Economic savings of $1,964 per patient achieved.
Conclusion
CONNACT model offers sustainable, evidence-based care pathways integrating hospital and community resources.
Lessons Learnt
Hospital-community partnerships and patient-centric approaches are key for success in managing chronic OA effectively.
