Aims
To improve access to medication assistance and ensure safe medication use through person-centered initiatives.
Background
Complex healthcare systems pose challenges for vulnerable patients needing medication support.
Methods
Developed workflows for cross-referral, conducted ESTHER cafes for patient feedback, provided transdisciplinary training, and delivered medications to community centers for co-management.
Results
Resolved 393 medication-related issues, facilitated 217 cross-referrals, and upskilled care providers with 22 learning sessions. Achieved continuity of care for vulnerable patients with safe medication use.
Conclusion
The CO-MED model facilitates collaboration among healthcare providers, improving medication management and patient outcomes in the community.
Lessons Learnt
Collaborative networks and community-based care models enhance patient outcomes and system efficiency. Addressing barriers helps provide person-centered care effectively.
