Aims
To redesign care through the Central Health LinkUp (CHL) system by providing automated e-triggers to community partners when their patients visit the ED, are admitted, or are discharged.
Background
Communication gaps between hospitals and community partners were affecting continuity of care, leading to uncoordinated care plans and readmissions.
Methods
Developed the Central Health Link (CHL), an automated system that triggers e-alerts to community care providers about patient visits to the ED, admissions, and discharges.
Results
Reduced readmissions by 8%, shortened inpatient length of stay (LOS) by 13%, and improved communication with 16 community partners.
Conclusion
The implementation of CHL significantly improved patient care, reduced unnecessary readmissions, and streamlined communication between the hospital and community partners.
Lessons Learnt
Collaboration with community partners was essential, and the need for a shared understanding of patient care goals was crucial.
Additional Information
The CHL system was successfully implemented in multiple NHG clinics and is being scaled to other areas for long-term sustainability.
