Aims
Equip carers in the community with health knowledge, skills, and confidence to better self-care and care for others.
Background
Community carers benefit from structured health programs tailored to build resilience and reduce care burden.
Methods
Five-module program utilizing a 70-20-10 learning model (experiential, social, formal learning) delivered online and in-person.
Results
619 carers trained across 36 centers. Modules enhanced care techniques such as medication packing and wheelchair transfers.
Conclusion
CHArge Up! improves community care capability, achieving better outcomes for residents and reducing re-admissions.
Lessons Learnt
Adapting to COVID-19 enabled hybrid learning. Continuous community support sustains program outcomes.
