Aims
Enable skilled institutional nurses to provide home nursing services to nearby patients.
Background
Singapore faces an ageing population; home-based care is critical to shifting care to the community.
Methods
Developed Match-A-Nurse app, recruited nurses and patients, and evaluated the programme.
Results
177 nurses participated; 56 patients benefited. High satisfaction from patients and caregivers.
Conclusion
Positive feedback validates expanding services and integrating with EMR for coordinated care.
Lessons Learnt
Collaboration with nurses and patients improved the app, ensuring functional enhancements.
