To ensure seamless transition from acute to community care, maintaining high rehabilitation standards through a co-learning framework and harmonized program.
Background
Rehabilitation for TKR patients is better suited for community settings rather than acute hospitals, enhancing cost-effectiveness and resource use.
Methods
A shared care model co-developed with community partners including structured co-learning, referral pathways, and harmonized rehabilitation programs.
Results
56 TKR patients referred to community centers. Plans include expanding partnerships and co-developing advanced rehabilitation programs.
Conclusion
Community-based TKR rehabilitation models improve quality of life and resource utilization.
Lessons Learnt
Community-based models ensure continuity of care, emphasizing the role of partnerships in successful rehabilitation.
