Background
Lack of a coherent Quality Framework led to fragmented committees, low staff QI competency, and poor patient safety indicators.
Methods
Adopted IHI framework; implemented leadership walkabouts, competency-based training, TQMC realignment, and CQI dashboards.
Results
QI-trained staff increased by 57%; falls reduced by 30%; CQI for Hip improved by 17% and Stroke by 15%.
Conclusion
SACH successfully implemented a framework that improved staff competency, patient safety, and CQI performance.
Lessons Learnt
Establishing a Quality Framework brings alignment, improves safety, and empowers staff to drive quality initiatives.
