Aims
The NCIS BCSP aims to right site low risk Breast Cancer Survivors (BCS) to the community.
Background
Despite BCS having excellent prognosis, they remain in tertiary institutions for annual surveillance.
Methods
BCS in remission after 5 years of active surveillance and no longer on treatment were referred to primary care providers with a Survivorship Care Plan (SCP) including mammogram and preventative health recommendations.
Results
There were substantial cost savings from transition of BCS care to primary care providers. Patients were surveyed post 1-year discharge for their satisfaction on a 5-point Likert Scale. Compliance with primary care providers visits, re-referral rates to the hospital and survivorship outcomes were tracked.
Conclusion
The NCIS BCSP demonstrates substantial cost savings through reduced cost to patients while optimising healthcare resource utilisation.
Lessons Learnt
The successful transition of care to primary care providers is evidenced by low re-referral and relapse rates and positive patient-reported outcomes.
