Aims
To provide optimal care for the index fracture episode and to prevent future fragility fractures through secondary prevention.
Background
Fragility fractures have a high mortality rate and require comprehensive management. The program aims to transform care through ED nerve blocks and expanded multi-disciplinary care.
Methods
The program encompasses inpatient, outpatient, and community care for all fragility fractures. It includes comprehensive screening, multi-disciplinary care pathways, and holistic management for secondary prevention.
Results
The implementation of nerve blocks in the ED increased from 66.7% in June 2024 to 100% in Jan-Mar 2025, providing effective analgesia and minimizing logistical delays.
Conclusion
The program shows promise in improving patient outcomes through enhanced peri-operative protocols and seamless transition support.
Lessons Learnt
Joint training and support by ED and Anaesthesia were enablers for the program. Barriers to prompt surgery were identified and addressed.
