Aims

To systematically implement Patient Reported Outcome Measures (PROMs) in NHG through digital integration to improve patient outcomes and enable data-driven decision-making.

Background

PROMs are essential for value-based healthcare, but their impact has been limited by fragmented data collection and poor integration with electronic medical records.

Methods

NHG embarked on a systematic approach involving planning, systematic integration, change management, and continuous improvement to embed PROMs into routine practice.

Results

Early results indicate improved capture of patient outcomes and enhanced clinical decision-making, with 9 additional PROMs questionnaires rolled out by July 2025.

Conclusion

NHG's approach demonstrates that digital integration and stakeholder engagement can overcome barriers to patient outcome measurement, establishing a sustainable framework for patient-centred care.

Lessons Learnt

Response rates remain modest due to factors like digital literacy. NHG introduced educational materials and incentives to improve participation.
