Aims
We aim to increase the percentage of Same Day Discharge (SDD) for patients undergoing elective Percutaneous Coronary Intervention (PCI) via Value Driven Outcome (VDO) program.
Background
Traditionally, patients require an overnight stay following PCI, leading to increased demand for high-dependency unit beds and longer waiting times. As of 2022, most elective PCI cases (more than 90%) in National University Hospital (NUH) are being discharged the following day. Paper published by American College of Cardiology with expert consensus had highlighted that patients can safely be discharged on the same day after appropriate monitoring, enhancing comfort and satisfaction while reducing healthcare costs.
Methods
The VDO framework provides a systematic approach to measuring healthcare costs and patient outcomes in the context of SDD for elective PCI procedures. This study employs a granular methodology that integrates cost accounting, outcome tracking, and benchmarking, ensuring that reductions in hospital stay duration do not compromise patient safety or satisfaction. The findings contribute to a broader understanding of value-based healthcare strategies and their application in optimizing procedural efficiency.
Results
The significant improvements in efficiency, cost-saving, and maintained high standards of patient care and satisfaction demonstrate the success of the SDD initiative. The dramatic increase in same-day discharges, from 8.9% to 76.1% of eligible patients, highlights the transformative impact of this program on PCI patient management. With an average cost saving of $4,100 per patient and a projected annual savings of $877,000 based on increased SDD uptake, this program directly addresses the need for sustainable, high-value healthcare delivery.
Conclusion
The success of this program lies in its low-cost, high-impact approach: leveraging existing resources, streamlining workflows, and transitioning to a nurse-led discharge model. This has minimized dependency on physician time and increased throughput efficiencykey factors in long-term financial sustainability.
