Aims
To achieve at least 80% of febrile patients who are developing new or worsening sepsis to receive antibiotic administration from baseline 220 mins to = 180 mins from fever recognition at Level 8 Wards Tan Tock Seng Hospital within 6 months.
Background
The problem involves delayed antibiotic administration in febrile patients developing sepsis, with a baseline median time of 220 mins. Prompt treatment is crucial, as highlighted by a study showing earlier antibiotic administration correlates with higher severity of illness.
Methods
Implemented PDSA cycles such as creating a Sepsis Care Bundle Workflow, conducting educational sessions, and sharing initiatives with healthcare teams. The intervention period was from September 2024 to March 2025.
Results
Post-intervention, 69% of patients met the =180 mins target, with a median time reduced to 147 mins. The intervention also resulted in a reduction in the median length of stay from 10 days to 8.5 days, saving 1.5 bed days per patient, translating to monetary savings.
Conclusion
The intervention was successful in reducing the time to antibiotic administration and length of stay, demonstrating the importance of structured workflows and continuous education in improving sepsis management.
Lessons Learnt
Challenges included difficulty establishing reliable baseline data, manual data collection burden, variable buy-in from departments, resource constraints, and the need for continuous education due to staff turnover.
