Aims
To reduce the time between referral and surgical intervention for dysfunctional PD catheters to less than 30 days, thereby minimizing patient costs and complications.
Background
Prolonged wait times for surgical intervention of PD catheters lead to financial burdens and disruptions for patients. A quality improvement project was initiated to address these issues.
Methods
A multidisciplinary team from the Renal and Urology departments was formed. They analyzed the patient journey, identified root causes of delays, and implemented a shared workflow and Microsoft Teams group to improve coordination.
Results
After interventions, 6 out of 7 patients had surgical interventions within 30 days, with a median wait time of 7 days. The shared workflow standardized the referral process and improved communication.
Conclusion
A standardized and shared workflow significantly reduced wait times for surgical interventions, improving patient care and reducing costs.
Lessons Learnt
The project highlighted the importance of standardized protocols and inter-departmental coordination. Initial outcomes are promising, but ongoing monitoring is necessary to ensure sustainability.
