Aims
This initiative aims to develop the integrated and coordinated patient scheduling for TA-Led intervention.
Background
In TTSH Integrated Care Hub (ICH), therapy assistant (TA) performs therapy intervention independently for occupational therapy, physiotherapy and speech therapy. The introduction of EPIC allows therapists to schedule TA-Led intervention into TA therapy timeslots through Cadence. However, scheduling TA-Led intervention requires appropriate processes and coordination. It may lead to inefficient therapist time use, that potentially delays patient care.
Methods
Since Jun 2024, TA-Led intervention scheduling system was redesigned through engagement of stakeholders (Therapists & TAs), development of TA-Led scheduling system, and enabling technological infrastructure. TAs self-schedule TA-Led intervention for patients with less complexity into their EPIC Cadence via EPIC Share List. Therapists directly schedule patients with higher complexity to selected TAs to perform TA-Led intervention, allowing flexibility.
Results
Since the implementation of the TA-Led scheduling system in Aug 24, the allocation rate of TA-Led through self-scheduling has improved from 67% in Aug 24 to 71.5% in Jun 25. TAs who were upskilled to self-schedule TA-Led has improved from 42% in Aug 24 to 93% in Jun 25. 62.5% of the surveyed TAs in Aug 24 reported TA-Led self-scheduling allows flexibility for them to schedule their workload, and 70.8% reported they are empowered by this initiative.
Conclusion
This initiative has been sustained and scaled into the routine workflow for scheduling TA-Led intervention.
Lessons Learnt
Collaborative approach with various stakeholders (TAs & therapists) enables problem-solving and enhances work processes. Self-scheduling TA-Led intervention may not be applicable for all patients due to their conditions (e.g., timing of medication, use of therapy equipment, patient's profile). Both TA Self-Scheduling and Therapy Open Market scheduling optimize the utilization of TA-Led intervention based on patient's condition.
