Aims

Background
We designed, implemented and evaluated a digital intervention utilizing patient-reported measures for patient health literacy and self-management in the Patient Reported Outcome Measures and Individualized Strategies using Electronic Feedback (PROMISE) program.
Methods
We designed, implemented and evaluated a digital intervention utilizing patient-reported measures for patient health literacy and self-management in the Patient Reported Outcome Measures and Individualized Strategies using Electronic Feedback (PROMISE) program.
Results
Twenty-six patients were included. Initial survey response rate was 50%, fluctuating between 28-50% at subsequent milestones. Responses were received for symptom self-monitoring (n = 84), EQ5D5L (n = 57), self-management resources (n = 42), and self-efficacy (n = 56). Patients reporting abnormal symptoms received telephone follow-up within one week, leading to early review for relapse and hospitalization in some cases. Requests for patient education and allied health professional consultations were also recorded.
Conclusion
A digital intervention utilizing telephone text message-based self-administered surveys can support patient self-management during immunosuppressive therapy for active glomerulonephritis.
