Aims
To catalyse weight loss in overweight/obese patients living with diabetes and improve glycaemic control, thereby reducing the risk of end-stage organ failure and diabetes-related complications.
Background
Lighter Life is a 6-month structured weight loss programme designed by NHG Polyclinics to provide a cost-effective intervention for glycaemic control in diabetes management.
Methods
Lighter Life employs a systematic patient detection and engagement approach, referring eligible patients to a 6-month structured intervention conducted in hybrid format. The programme integrates dietetic education, exercise, behavioural modification, peer support, and goal-setting strategies.
Results
In the review period of Jan 2023 to Jan 2025, 94% of patients completed the programme. 73% achieved weight loss, 25% achieved clinically significant weight loss of 5%, and 16% sustained 5% weight loss at week 48. Patients with diabetes achieved a mean HbA1c reduction of 0.5%.
Conclusion
Weight loss is an important cost-effective intervention for diabetes management. Lighter Life is effective for overweight/obese patients with diabetes, with further cost-savings achievable as enrolment scales.
Lessons Learnt
The cost-effectiveness of Lighter Life will improve as enrolment scales, but current lack of local data on cost savings may impede enrolment.
