Analysis of TTSH data from January to December 2023 revealed 1,951 cases of insufficient medication prescriptions across all departments, with the CVM Clinic accounting for 374 cases.
Background
Analysis of TTSH data from January to December 2023 revealed 1,951 cases of insufficient medication prescriptions across all departments, with the CVM Clinic accounting for 374 cases.
Methods
A multidisciplinary team was formed to address the challenge using CPIP methodology. Diagnostic tools such as flowchart, brainstorming session, fishbone diagram, and Pareto Chart were used to identify root causes. Interventions were implemented through Plan-Do-Study-Act (PDSA) cycles.
Results
By December 2024, there was a reduction in insufficient medication prescriptions from an average of 30 to 24 cases monthly. This resulted in significant time savings and cost savings for the pharmacy department.
Conclusion
The initiative represents a paradigm shift in patient care, with streamlined processes empowering healthcare providers, enhancing patient trust, and renewing commitment to excellence.
Lessons Learnt
The standardized workflow and improved communications between doctors and patient service associates help prevent insufficient medication duration until the next doctor appointment
