Aims
To decrease the Integrated Diabetes Care Programme (IDCP) Clinic First Visit default rate from 60% to 30% within 6 months.
Background
Singapore has a high incidence of diabetes mellitus (DM) among developed countries. Many patients are admitted in Tan Tock Seng Hospital (TTSH) for DM-related complications. The IDCP clinic functions like a post-discharge DM early review clinic
Methods
A team was assembled to map out the inpatient IDCP review and transition to outpatient IDCP clinic. A pilot survey was conducted among patients who defaulted. A 'Discharge Communications Bundle' was created and implemented.
Results
The intervention led to an improvement in the IDCP clinic FV default rate from a pre-intervention median of 66.6% in December 2023 to a post-intervention median of 44.4% by July 2024.
Conclusion
The 'Discharge Communications Bundle' intervention was effective in reducing the default rate, leading to better resource utilization and potential cost savings.
Lessons Learnt
Healthcare is increasingly complex, requiring coordination of many services. Systems designed with appreciation of patient challenges can lead to better care outcomes and less waste.
