Aims
To improve partial compliance of home exercises for patients attending Physiotherapy in TTSH Centre for Geriatric Medicine (CGRM) at their 2nd follow up appointment from a baseline of 32.5% to 70% (Stretch goal: 80%) in 6 months.
Background
Low exercise compliance is a significant issue, with only 32.5% of CGRM patients being more than 50% compliant with prescribed exercises. Home exercises are the main mode of intervention for CGRM outpatient patients, and the efficacy of therapy relies on patient compliance.
Methods
A multidisciplinary team applied the Clinical Practice Improvement Programme (CPIP) methodology, using PDSA cycles to implement interventions aimed at improving exercise compliance.
Results
The percentage of patients with more than 50% exercise compliance improved over the period from 22 Jan to 15 Nov 2024. Cost savings were also achieved, with an average cost of therapy services 'saved' per patient being $9.66, leading to a total annualized cost saving of $18,547.20.
Conclusion
Interventions should be feasible and tailored to patient needs. Exercise compliance should be a key indicator for therapy success, and continual education and identification of champions are crucial for sustaining improvements.
Lessons Learnt
The problem statement was broad, requiring refinement. Behavior change is complex and multifactorial, making it difficult to have a single tool that suits everyone.
Additional Information
Root cause interventions included exercise planners with video narration and group sessions to tap into peer support. Problems encountered included the broad scope of the problem statement and the complexity of behavior change.
