Aims
The goal is to improve patient care and experience by setting up value-driven care in SLH, tailored to the CH setting.
Background
St. Lukes Hospital (SLH) is a 223-bed community hospital in Singapore, with a diverse ecosystem of inpatient, outpatient, and home care services. Previously, there was no system in place to measure condition-specific value created for patients across various services.
Methods
There was an 11.4% relative year-on-year increase from 31.7% to 35.3% in CQI. Since the establishment of IPU teams, more than 15 IPU-driven initiatives and workflows were implemented, enabled by timely data access from IT. Implementation costs were largely manpower, with clinical staff at 2.5 FTEs/year and administrative staff at 2.3 FTEs/year over FY2023 - FY2024.
Results
Data was collected for FY23 (Apr 2023 - Mar 2024) and FY24 (Apr 2024 - Mar 2025) to enable year-on-year comparison. There was a clear overall YoY increase in CQI.
Conclusion
VBHC has been successfully implemented in SLH with a clear overall YoY increase in CQI. The formation of IPUs has allowed clinicians to target condition-specific outcomes related to their area of interest.
Lessons Learnt
The formation of IPUs has allowed clinicians to target condition-specific outcomes related to their area of interest. Future efforts will focus on broader adoption across more conditions and sustaining effective IPU-driven initiatives.
