Aims
To implement system-wide value-based transformation in asthma care within NHG Health.
Background
Singapore reports a 10.5% lifetime prevalence of asthma and one of the highest age-sex standardised asthma hospitalisation rates among OECD countries, resulting in significant burden from exacerbations and lost productivity. NHG Health historically managed asthma care independently across institutions with varying clinical practices and outcome measurements.
Methods
The NHG Respiratory Health Steering Committee (RHSC) Asthma Workgroup, comprising clinical champions across NHG Health institutions, drove systematic changes to develop a cluster-level value framework and standardised care practices, supported by NHG Value-Based Care Team and PopHealth.
Results
The standardised value-based approach to clinical practice, data collection, and cost measurement positions NHG to optimise asthma care at system-level and achieve improved outcomes. ~80% of NHG Health patients are managed in primary care, with tertiary care focusing on complex asthma care management.
Conclusion
Implementation of the value-based care (VBC) framework has promoted understanding of population-level asthma management, emphasising primary care as the cornerstone of effective asthma management.
Lessons Learnt
Despite challenges in reconciling institutional legacies and varying clinical practices, the NHG RHSC Asthma Workgroup successfully established a unified VBC framework across 12 institutions within two years through a collaborative approach.
