Aims
To evaluate the clinical outcomes, patient satisfaction, and cost savings of a pharmacist-led dupilumab monitoring service.
Background
Dupilumab is an effective and well-tolerated biologic for moderate-severe atopic dermatitis (AD). Common adverse reactions in the Asian population include ocular symptoms and head and neck dermatitis. The Pharmacist Eczema Clinic (PEC) implemented a dupilumab monitoring service to review patients for potential adverse reactions 2 to 4 weeks after initiation.
Methods
A retrospective chart review was conducted at the National Skin Centre on patients who started on dupilumab between 31 July 2020 and 4 Jan 2023. Outcomes of those reviewed by PEC were compared with those seen by dermatologists.
Results
The pharmacist-run dupilumab monitoring service demonstrated satisfactory clinical outcomes and high patient satisfaction at a lower cost to patients. Sample: 72 patients (36 patients per group), baseline demographics comparable. DRPs addressed: Dermatologist 4 vs PEC 5. 28-day dermatology-related walk-ins: Dermatologist 3 vs PEC 0. Patient satisfaction: 22/23 PEC patients (96.5%) rated care as very good or excellent. Cost: PEC manpower cost/patient: SGD 13.93, Average PEC consultation fee (after subsidy): SGD 19.94, Average direct cost savings for patients reviewed by PEC vs dermatologist: SGD 14.92 (42.8% reduction in consultation fees).
Conclusion
The pharmacist-run dupilumab monitoring service demonstrated satisfactory clinical outcomes and high patient satisfaction at a lower cost to patients.
Lessons Learnt
The pharmacist-led service can effectively manage drug-related problems and reduce dermatology-related walk-ins while maintaining high patient satisfaction and reducing costs.
