Aims

Background
Hospital at Home (HaH) delivers acute inpatient care to patients in their homes. Nursing home (NH) residents often require hospitalisation during acute deterioration and may be suitable for HaH admissions.
Methods
NUHS@Home partnered with a NH group to allow admissions to HaH for residents requiring inpatient treatment. Screening of residents admitted to acute hospitals identified residents suitable for care under NUHS@Home. Residents are reviewed up to thrice daily for intravenous medications or nursing procedures with daily physician review by the NUHS@Home team.
Results
Between May 2024 and April 2025, 34 out of 120 eligible NH residents were admitted to NUHS@Home. There was no significant difference in 30-day readmission and 30-day mortality between admitted and eligible but not admitted residents. NUHS@Home saved 125 bed days with an average HaH length of stay of 2.9 days. Average cost of admission under NUHS@Home was 17% cheaper compared to eligible non-admitted residents.
Conclusion
Hospital at Nursing Home is a safe service delivery model that reduces length of stay in acute hospitals at a lower cost.
Lessons Learnt
Engagement sessions led to a 240% increase in admission rates. Collaboration with NTFGH ED is underway to redirect NH residents needing hospitalisation directly to NUHS@Home.
