Aims
The aim of the value programme evaluation framework is to create value at the programme & systems level by assessing opportunities and developing enablers for value-based healthcare.
Background
Singapore's healthcare system faces mounting challenges from an aging population and rising costs, creating an urgent need to shift from volume-driven to value-driven care. Tan Tock Seng Hospital (TTSH) developed a systematic framework to evaluate value-based healthcare programmes.
Methods
The evaluation framework draws inspiration from Institute of Healthcare Improvements Triple Aim and Prof Michael Porters Strategic Agenda. Programmes receive scores from 0-3 in each component, with total scores determining their maturity stage from early-stage to fully mature.
Results
In a pilot evaluation of 33 programmes, the framework revealed that 42% were at intermediate/developing stage while 58% had reached fairly mature status.
Conclusion
TTSH's journey towards value-based healthcare requires seamless care coordination across healthcare settings and enhanced post-discharge monitoring systems. A comprehensive assessment method will provide deeper insights into each programme's effectiveness and value.
Lessons Learnt
The assessment identified several systemic gaps, particularly in extending care beyond hospital walls, maintaining robust cost evaluation systems, and establishing comprehensive data infrastructure for outcome tracking across different care settings.
