Aims
Transform nutrition care from hospital-based to community-based preventive care, aligning with Healthier SG and Age Well SG initiatives.
Background
EatWise SG addresses critical gaps in nutrition care such as inadequate access to dietitians, delayed identification of malnutrition, fragmented care pathways, and cost-logistical barriers.
Methods
Implemented across 6 major community partners between Apr 2024 to Mar 2025 in NHG Health Central-North regions of Singapore. Implementation stages include engaging major partners, differentiated training curriculum, adoption of standardized resources, and standardizing risk stratifications.
Results
Over 500 non-dietitian providers were trained with 90% reporting training as useful. 50% improvement in providers' confidence in providing diet recommendations post-training. Maintained 80% actualization rate for Tier 3 malnutrition referrals within 3 weeks. 83% of residents expressed satisfaction with care quality.
Conclusion
EatWise SG demonstrated effective structuring and delivery of community-based nutrition care, improving access equity beyond hospital settings. Future plans include nationwide scaling, enhancing digital integration, and strengthening partnerships.
