Aims
To assess medication adherence and evaluate the quality of healthcare delivery from the patient's perspective using Patient-Reported Outcome Measures (PROMs) and Patient-Reported Experience Measures (PREMs) in a multidisciplinary CKD clinic.
Background
Medication adherence is critical in CKD management due to polypharmacy and complex regimens. High pill burden and side effects can lead to suboptimal adherence, worsening disease outcomes, and increasing hospitalizations. Traditional measures like refill records may not capture patient-specific barriers.
Methods
PROMs and PREMs were used to assess medication adherence and healthcare quality. The study included CKD patients in stages 4 and 5, excluding those in nursing homes, with life-limiting malignancies, over 80 years old, or under active immunosuppression. The time period was from July 2022 to December 2024.
Results
Medication adherence improved across successive clinic visits. Nearly all patients reported feeling more confident in managing their medications. PROMs identified common barriers to adherence as forgetfulness, being out of routine, and being too late with doses.
Conclusion
Routine use of PROMs and PREMs effectively identified barriers, improved adherence, and enhanced patient satisfaction in a multidisciplinary CKD clinic.
Lessons Learnt
The integration of PROMs and PREMs adds value beyond traditional adherence measures by revealing patient-specific barriers and providing direct insights into healthcare quality from the patient's perspective.
