Aims
To improve acute care outcomes and demonstrate cost avoidance through the ischemic stroke Value Driven Care (VDC) initiative at Singapore General Hospital (SGH).
Background
The initiative began in 2020 and adopted the International Consortium of Health Outcome Measurement stroke standard set (ICHOM-SSS) to extend care considerations beyond acute care and incorporate patient-reported outcomes (PROMs).
Methods
The methodology involved mapping variables for VDC with the ICHOM-SSS, patient journey mapping, ground immersion and engagement, and designing data collection processes. PROMs were implemented at Rehabilitation Medicine (RMD) and Outram Community Hospital (OCH).
Results
Of the 24 ICHOM-SSS variables, 12 were collected under VDC, 6 were retrievable from SGHs data warehouse eHIntS, while the remaining variables required workflow redesign. Six discharge pathways were identified, each involving different staff groups.
Conclusion
ICHOM-SSS implementation requires groundwork to assess feasibility and obtain buy-in. The expanded SLO service represents a possible solution for using PROMs in clinical care, although further evaluation of outcomes is underway.
Lessons Learnt
Successful implementation requires understanding current systems to identify data sources and stakeholders to engage. Coordination with all relevant stakeholders across disparate discharge pathways was a challenge, leading to a decision to pilot with two settings.
