Aims
To address antibiotic over-prescribing in LRTI by implementing AI2D to expand prospective audit feedback of selected antibiotics and identify inappropriate antibiotics for discontinuation without compromising patient outcomes.
Background
Lower respiratory tract infection (LRTI) is a common infection globally and a leading cause of hospitalization. A significant percentage of antibiotics used for LRTI are unnecessary, highlighting the need for better oversight and prescribing practices.
Methods
AI2D was developed and implemented by the SGH Antimicrobial Stewardship Unit to assess antibiotic courses for LRTI. It underwent technical and clinical validation studies and was integrated with SGH electronic medical records.
Results
AI2D assessed 658 antibiotic courses, flagging 31.1% for review. ASU interventions stopped antibiotics in 40% of patients, with significant cost savings and no compromise in patient safety.
Conclusion
AI2D represents a significant advancement in combating antibiotic resistance and improving infectious disease management, with potential for wider adoption and further improvements in healthcare delivery.
Lessons Learnt
AI2D demonstrated the potential for reducing unnecessary antibiotic use and improving clinical workflows, though ongoing validation and adjustments are necessary for broader implementation.
