Aims
To reduce avoidable ED visits and admissions, preserve acute hospital capacity for critical patients, and keep patient care in the community.
Background
Demand for acute hospital resources is at an all-time high, leading to overcrowding in Emergency Departments and delayed patient care. Alexandra Hospital, like many others in Singapore, faces high patient volumes and occupancy.
Methods
The Alexandra Hospital Virtual Care Centre (AH VCC) was set up to provide tele-triaging and medical advice to discharged patients and caregivers, aiming to reduce unnecessary visits and re-attendance to EDs and UCC.
Results
From Aug 2020 to Dec 2022, 2,543 inpatient bed-days were saved, and a total inpatient bill of $2.06 million was avoided. The utilization rate increased by 3.5 times since launch, with an average of ~10% of discharged patients utilizing VCC.
The re-admission rate of AH patients showed a downward trend post-implementation, with rates decreasing from 11.45 in 2019 to 7.64 in 2022.
Conclusion
The AH VCC successfully reduced re-admission rates and preserved hospital resources, aligning with the Ministry of Healths strategy of keeping care in the community.
Lessons Learnt
Tele-triaging and right-siting of care can effectively reduce unnecessary hospital visits and improve patient satisfaction, as indicated by a satisfaction rating of 8.6/10.
