Background
Variability in skin care practices led to ineffective prevention and management of pressure injuries.
Methods
Conducted training, implemented SKIN Care Bundle in pilot wards, expanded hospital-wide.
Results
Reduced PI incidence by 72.7% (from 0.11 to 0.03 per 1,000 patient days); cost savings of SGD $7,408/year.
Conclusion
The SKIN Care Bundle significantly reduced pressure injuries and improved staff confidence.
Lessons Learnt
Multidisciplinary collaboration and continuous training are critical for project sustainability.
