Aims

Provide integrated inpatient and outpatient care with One Care Team, One Bed, and One Principal Doctor.

Background

AH was set up as Singapore's first Integrated General Hospital (IGH) to address care fragmentation and promote consolidation.

Methods

Implemented IGH care model, including inpatient tiers, telehealth (vCare), Virtual Care Centre (VCC), and One Principal Doctor system.

Results

45% of patients were down-triaged to appropriate care levels; 40% avoided transfers to subacute facilities; cost savings of $893,091.

Conclusion

IGH care model significantly reduced care fragmentation and increased efficiency and satisfaction.

Lessons Learnt

Multidisciplinary collaboration and empowerment of nurses were critical for the IGH model's success.
