Current tertiary care models fail to address long-term needs of cancer survivors effectively.
Methods
Implemented routine distress screening and built a community network for post-treatment support.
Results
Reached 79% eligible patients, improved psychosocial support access, reduced emergency visits, and hospitalizations.
Conclusion
Enhanced survivor care through community integration, reducing healthcare utilization and improving quality of life.
Lessons Learnt
Continual staff engagement, usability data, and alternative tools are necessary for sustainability and wider adoption.
