Aims
To improve Palliative Care Outcomes Collaboration (PCOC) completion rates in home hospice visits.
Background
PCOC completion rates were inconsistent due to workflow inefficiencies and lack of standardization.
Methods
Conducted two PDSA cycles: standardization/training in Cycle 1 and reinforcement/reminders in Cycle 2.
Results
Improved PCOC completion rates from 68.8% to 91%, sustained for 12 months.
Conclusion
PCOC compliance ensures better palliative care delivery and enhanced continuity of care.
Lessons Learnt
Standardization, training, and stakeholder engagement are key for sustainable quality improvement.
