Aims
Reduce lead time to first contact of Phase II Cardiac Rehabilitation.
Background
Long lead times and underutilization of allied health resources required process improvement. The MDC model was not meeting demand effectively.
Methods
Root Cause Analysis (RCA), Plan-Do-Check-Act (PDCA), and establishing a joint clinic with nurses and physiotherapists.
Results
Lead time reduced from 52 to 14 days; increased patient participation in Phase II rehab, saving $60 per patient.
Conclusion
Joint nurse-physiotherapist clinics enhance cardiac rehab service quality and reduce resource strain.
Lessons Learnt
Multidisciplinary collaboration and streamlined workflows maximize resource utilization and reduce delays.
